If you are disabled or in ill-health and finding it hard to manage at home, you can request a community care assessment from your council's social work department. A community care assessment is a review of your personal circumstances and needs. Social work will look at how you cope with day-to-day living, and recommend help or equipment that might make life easier for you. You can find out more about care assessments here.
